# Creating PDF Forms in Illustrator CS3?

Hi all. I’ve created a gift voucher in Illustrator CS3 and have converted it to a PDF. There are 4 fields that need to enable the viewer to type in information. How can I create forms in an illustrator file that will allow the user to type in them and print them for that matter, once it’s converted to a PDF? Thanks!
