I am trying to to add a checkbox to a mailform that uses PHP. But I cannot make it work.

All I need is a checkbox that needs to be checked before the email can be sent(for terms & conditions) and I was hoping that someone can do it for me.

The files(FLA & PHP) can be found here

If someone can help me out, the fixed filed can be emailed to [scubamartin2@gmail.com](mailto:scubamartin2@gmail.com)

THANKS!!!
