# How create form with attachment

Hello ,

I would like create small FORM with attachment.

- name
- city
- massage
- and button with attachment (only documents \*.pdf and \*.doc)
- button send.

It is possible, than user send to me massage from Flash, admin get email with attachment ?

I find in this forum, easy Form but without attachment ☹  
so my problem is how create additional function attachment.  
Could You help me ?

P.S. Sorry but my english is bad.
