# How to make form with multiple choices

kindly provide me links or tutors or books or fla files (source files)

for making a form which contains multiple choice questions and a total number of questions  
attempted total number of questions answered correctly and questions answered wrongly etc

and also if a user takes four subject exams then we want individual total and also

total marks secured
