# Lame request - building an easy form

Hi,

I’m building a website which needs to have a simple type of form so that people can submit their personal information and choose among a few options.

Do you know where I can find a tutorial to build one that includes:

[size=1]normal input text boxes, ComboBoxes, ListBoxes, Checkboxes, and Radio Buttons[/size]

a way to send the filled\*\*/\*\*selected information to two email adresses (one is the receiver, me, the other one is the user’s email adress, submited in the form)

A Submit and a Reset Button.  
&  
It can’t be submitted without filling in all the required fields, with a verification mark in case one or more fields weren’t properly filled (or comboboxes submited with the blank option).

Also…

How can I slightly customize the default look of these components (comboboxes, listboxes, etc), such as the selection & highligh colours ?
