# Need help creating comment form

I saw this comment form on [Youtube.com](http://Youtube.com) and million different websites so i want to add  
to my homepage but just cant seem find a tutorial on how to create this type of work  
it looks like this

can anyone help me or guide me to a good tutorial on how to build this type of  
comment form. Any help would be appreciated!
