# PHP and Flash

I am trying to build one flash contact form with php but it has not working properly . The fla is very big to upload so I am only attaching the php form for your look. Can any make it working.

There is a send and cancel button. Rest is as usual. Can any body give me where I am going wrong.

please advice me.
