I’m trying to create an online form for a book publishing company. It does not involve payment. Rather, order information sent to client’s email.

I’ve been looking at php/html/flash codes for email forms, but i’m looking for something more like this:

Does any one know where I can find a php/html code that involves, radio, check boxes, qty selection or drop down?

Thanks in advance!

Sam
