# PHP form

Hey everyone. I am at a complete loss here. I’ve got this form to work on several other websites but can’t seem to find the problem here. Here is the address:

and the “thanks” screen prompt for errors

Am I using the wrong kind of server? Linx vs. ASP?  
I don’t know how to fix this at all?  
Can someone who knows PHP help me?
