# PHP script advice needed

having an issue with my flash form…i can create a form with 3 main text areas (name, email, message) but i need to add 6 more items in my script…

1.name  
2.email  
3.message  
(not a problem)

4.pickup-city  
5.pickup-state  
6.pickup-zip  
4.delivery-city  
5.delivery-state  
6.delivery-zip
