I have a client who would like a quiz on her site that follows this format:

1 2 3 4 Question goes here

1- Strongly Disagree
2- Disagree
3- Agree
4- Strongly Agree

At the end of the quiz, it tallies your points and tells you how you do.

I’ve seen this on [Quizilla.com](http://Quizilla.com) but it’s an external thing; I’m looking to do it self-contained. I assume it uses JavaScript.

Anyone know how to make one? Or perhaps link to a tutorial showing how?

Thanks 🙂
