Well I solved my problem and thought I’d provide my completed form to share with others. It has text fields for Name, Email address, Tel, Subject and Message and will generate an auto reply to the originator once sent.

The attached zip file contains the fla, swf and php files. Note you will need to change some details in the php file so that the mail returns to your address.
