Flash - maybe some after effect’s with some siwft 3d, just a guess
gotoandlearn.com has a really good tutorial on creating a flash button in after effects. It is in AS3 tho.

Combustion is pretty good. I lay my hand on this software 3 years ago. It takes little more time to know all the functions but the final layout is just awesome.
yes. but not only. blur/fades, 3d zooming, transition effects, button rollovers, logo animations, etc. They are very nice and smooth.
