Help with kirupa email php contact form

I used the kirupa flash/php email form on my site. Only trouble is the user’s name does not get sent to me. I receive an email that gives the the content of the “comments” field, and I can see the sender’s email address, but nothing from the “name” field. Can anyone please help me???
