How create form with attachment

Hello ,

I would like create small FORM with attachment.

  • name
  • city
  • massage
  • and button with attachment (only documents *.pdf and *.doc)
  • button send.

It is possible, than user send to me massage from Flash, admin get email with attachment ?

I find in this forum, easy Form but without attachment :frowning:
so my problem is how create additional function attachment.
Could You help me ?
