How do I do make this on dreamweaver? Do I need to know php? How do I set it up?

what a feedback page?
you dont really have to learn php, yes therez an outside supportive file in php format, usually it has 2 functions to carry forward.
one > send the information filled to your email account.
two > redirect the user to home/thanks page after filling up the form.

you can make the form in dw or frontpage and can add validations thru java script which are easily available, search google.

I have tried to do a search and I have found some scripts but none that I have used worked. It always gave me an error. Do you have any links to sites that have a tutorial on it, and that also work?
