[NEEDED] ASP, programmer to do some form work

This job will be paying 250.00 usd. I need someone reliable and fast. No design work will be needed. Currently there are 3 forms, they need to be combined into one using checkboxes. For each checkbox the user clicks another part of the form will show up. There needs to be an upload utility that will let the user upload attachments to a FTP server. And a unique ID needs to be ammended to the name of that file, plus that ammended filename will be sent to a email. I could do this myself but I am being occupied by other portions of the project.
