PHP Form - how to add seperated form fields to output

Hi All. I found the following script to process my email form but it only displays the enquiry field from my form whereas I need the name, company, phone, email and enquiry fields displayed on the email message and displayed logically. I would appreciate anyone having a quick look through the script and point me in the right direction. I don’t have a clue what I am doing and have been fumbling around trying to get it to work. Here is the script and I have marked my changes in red:
