PHP Form - http://yoce.com/f.html

I am trying to get this form to go to an error page if not filled in corectly which lists each error, ie. ‘please enter a valid email address’, ‘please select policy type’ etc etc…

please can someone help as i have no experience with editing php files and don’t know where to start
