Senocular's email.php for Flash contact form

I know I implemented Senocular’s Flash form php script straight out of the box correctly, as I received a test email.

However when I added 2 more fields, I stopped receiving form emails

I thought I modified the script correctly. Things that I changed or added in red. My new fields have variable names phone & zip
