Submiting Text Fields

Hi guys,
Probably a simple one for someone. I am using a simple PHP mailer form in Flash MX, I have set the form up from one of the tutorials and it works fine, but I need to include other fields that I have in my movie, i.e. at the moment it sends and e-mail with name, e-mail add. etc and a message. I also need it to send 2 other text fields that have been setup with variable names.
This is the PHP I’m using:

How do I get it to send 2 other text fields I have from flash?
i.e. I have 2 more input fields, ‘telephone’ and ‘company’ that need to be sent with the e-mail
