Upload .jpg/.gif to db(sql)

can some one please help me… im a newbie to php…

how do i script a page with 2 fields one to upload files(image files) onto a database(sql ) and one for a url …

and then get the image from the database and then show it on another page and have the image linked to that url…?

I have 4 fields… need to upload 4 files at a time( multiple uploads )
how do i do that?
