Dreamweaver - email set up

Hi can anybody please help me. Same problem as my previous (if you read that) just now I want to know how to do it in dreamweaver.

Was wanting to set up some user input text fields for emailing so I would like to have 3 text fields:

  1. would be the users name
  2. would be the users email address
  3. would be the users message
    and then I would like to have a submitt button or something similar to send the info to me.
    can anyone please help me with this?
