Interested in having somone help me set up a databased, PHP interest list form. probably 10-15 questions. results would be stored in a mysql database that could be accessed through a secure webpage & downloaded to excel. I will code the inital form page in html/css, I just need an experienced, proffessional to connect the dots for me. Must also be available for follow up if the client need’s changes down the road.
