I’d like to refer a customer of mine to one of you, he’s in need of a simple script, preferrably in javascript that could be embedded easily into an html page. It would require the form code and functioning script that would calculate shipping costs, similar to the example below:

obviously the rates and values would be his own,
or if you know of a service that provides this type of code that would be great as well…
